Still Tired on TRT? Here's What to Look for in Your Bloodwork Before Blaming the Testosterone

Testosterone in range and still exhausted is one of the most common TRT complaints — and usually the answer isn't a dose change, it's somewhere else in the bloodwork nobody looked at yet.
"My testosterone is in range and I still feel exhausted" is one of the most common frustrations reported on TRT, and the instinct is usually to blame the dose or the protocol. Often, the real answer is sitting somewhere else entirely — in bloodwork that either wasn't ordered in the first place, or was ordered but never really examined beyond the testosterone number itself. Here's where to actually look.
Start With What's Already Being Checked, But Read Differently
Trough testosterone timing: If bloodwork is drawn right after an injection rather than immediately before the next one, the number reflects a temporary peak, not what your body is running on most of the week. Thyroid panel (TSH, free T4): Thyroid and testosterone symptoms overlap almost completely — fatigue, brain fog, low mood, cold intolerance, weight changes. A standard TRT follow-up panel frequently checks testosterone, estradiol, and hematocrit, but doesn't always include thyroid markers unless specifically asked for. Given the symptom overlap, this is one of the highest-value, lowest-cost additions to a fatigue workup on TRT.
Vitamin D: Worth checking, though it's fair to be precise about what the evidence actually shows rather than overselling it. How to Actually Approach This
The practical sequence: confirm the testosterone level itself reflects a proper trough draw, not a post-injection peak. Persistent fatigue on TRT is a legitimate, common complaint — but "the testosterone dose must be wrong" is often the wrong first conclusion.
